STATE OF NEW HAMPSHIRE

In the Year of Our Lord Two Thousand Seven

AN ACT relative to advance directives for durable powers of attorney and living wills.

Be it Enacted by the Senate and House of Representatives in General Court convened:

1 Use of Statutory Forms. Amend RSA 137-J:13, II to read as follows:

II. An advance directive executed on or after the effective date of this chapter shall be substantially in the form set forth in RSA 137-J:20 or other form desired by the principal.

2 Forms for Advance Directives. Amend the introductory paragraph of RSA 137-J:20 to read as follows:

An advance directive in its individual “Durable Power of Attorney for Healthcare” and “Living Will” components shall be in substantially the following form or other form desired by the principal:

3 Effective Date. This act shall take effect upon its passage.
