I.

Applications for licensure made using the method prescribed and furnished by the office of professional licensure and certification.

Applicants shall include the following:

Proof of required education.

(b)

Proof of passing the eligibility testing requirements.

Payment of applicable fees.

Proof of liability insurance.

Any other attachments as required by the board.

III.
