436:51 Contents. –
The report shall include, when the test is made by the subcutaneous method, a complete temperature record and such other information as the commissioner may require, and, when said test is made by either the intradermal or ophthalmic methods, a complete record of such reaction or reactions as appeared, and such other information as the commissioner may require.
