REQUESTED ACTION
Authorize the Governor s Office of New Opportunities & Rural Transformational
NORTH) to enter into a Memorandum of Understanding (MOU) with the Department of Hea
and Human Services (DHHS) for the administration of the Rural Health Transformation program
effective upon Governor and Executive Council approval through September 30 203
EXPLANATION
This MOU sets forth the roles, responsibilities and expectations regarding the
Rural Health Transformation grants funds from the Centers for Medicare and
(CMS) between GO-NORTH and DHHS This MOU formalizes the relationship in accordancewith the provisions set forth in Executive Order 2025-06

Cipportunities and Rural Translbnnational Health (GO-NORTH) and the Nexx Hampshire Department ol
Health and Human Services (DHHS) sets forth the parties understanding with regard to duties and
isbligations.
The parties to this Memorandum of Understanding herebx agree as folloxxs:
As artieulated in Fxecutixe Order 2025-06. the Goxernor's Otllce of New Opportunities and Rural
Transformational Health (GO-NORTH) shall haxe the power and responsibilities to assist the Governor
with the management and expenditure of Rural Health Transformation funds. Recognizing the relationship
established bv this executive action. GO-NORTH and Department of Health and Human Serv ices (DHHS)
collectivelv commit to ensure operatkms consistent with the Governor's direction: DHHS—the C MS-
recoenized cognizant agencv—is directed to act consistent with New Hampshire s RHF application and
izrant as operated bv GO-NORTH, and GO-NORTII is likew ise directed lo act consistent w ith all applicable
CMS obligations for RHT funds.
