Safety (DOS) and the Department of Transportation (DOT), to enter into a sole source amendment
to contract DOS 2022-036/DolT 2022-036withCoforgelnc. (\/C#165248), Princeton, Nj, increasing
the price limitation by $551,830, from $11,745,545 to $12,297,375, to develop a Crash Data Portal
for towns without Records Management Systems (RMS). Effective upon Governor and Council
approval: no change to the contract end date of August 31,2027. Source of funds: 100% Federal
(HSIP via DOT), as detailed below
The contract was originally approved by Governor and Council on August 17,2022, Item #27, and
amended by Amendment A on September 21, 2022, Item #39, Amendment B on March 22, 2023,
Item #38, Amendment C on September 20,2023, Item #66, Amendment D on January 10, 2024,
Item #27, Amendment E on April 23,2025, Item #38, and Amendment F on December 17, 2025,
Item #25. The completion dates for the Original Contract, Amendment A, Amendment B,
Amendment C, Amendment D, Amendment E and Amendment F will remain unchanged as
shown in Contract History of this Amendment G. There is no extension of the overall contract
completiondate of August 31, 2027. No General Funds are requested for this action; this effort will
be funded by the NH DOT. Source of Funds: 100% Federal Highway Safety Improvement Program
(HSIP) Funds.

This request is designated sole source under MOP 150 IX(A)(4) because it amends the scope of
services and requests additional funds to add a new deliverable (Crash Data Portal) to the existing
contract. The original contract was competitively procured under RFP DOS 2022-036 and approved
by G&C on August 17.2022, Item #27, with subsequent Amendments A-F as noted. The purpose of
this Amendment is to develop a crash data entry portal for towns that do not have an existing
means to submit crash data electronically. The Agency's Data Warehouse and eTicket platforms
are already managed by Coforge, as the Crash Data Portal must integrate with their established
"Innovative Technologies Today for New Hampshire's Tomorrow"

data models, ETL pipelines, APIs, logging, and security controls—using the incumbent preserves
architectural continuity, maintain security/data quality, and accelerate delivery. By providing a
portal for these towns to submit crash data electronically, we will gain comprehensive crash data
insight across NH for input into the predictive Crash Data Analysis System.
The Department of Information Technology requests approval of this contract amendment with
Coforge, Inc.

This Amendment (the "Amendment") is entered into by and between the State of
New Hampshire, acting through the Department of Information Technology (the
"Department") and Coforge, Inc. and 502 Carnegie Center Drive, Ste 301,, Princeton, NJ,
08540, USA ("Contractor"). The Department and Contractor are sometimes referred to
herein singularly as a "Party" and collectively as the "Parties."
WHEREAS, pursuant to an Agreement approved by Governor and Council, as a
result of RFP DOS 2022-036, on August 17,2022, Item #27, amended by Amendment A on
September 21,2022, Item # 39, Amendment B on March 22,2023, Item #38, Amendment
C on September 20, 2023, Item #66, Amendment D on January 10, 2024, Item # 27,
Amendment E on April 23,2025. Item #38. Amendment F on December 17,2025, Item #25
(herein after referred to as the "Agreement"), Coforge Inc. (Contractor #165248;
hereinafter referred to as "Contractor") agreed to supply certain services upon the terms
and conditions specified in the Agreement and in consideration of payment by the
Department of Information Technology (hereinafter referred to as the "Department"),
certain sums as specified therein;
WHEREAS, pursuant to the Agreement Section 17: Amendment and the provisions
of the Agreement, the Agreement may be modified or amended only by a written
instrument executed by the parties thereto and approved by the Governor and Executive
Council;
WHEREAS, the Contractor and the Department have agreed to amend the
Agreement in certain respects;
WHEREAS, the Department wishes to increase the contract price in the amount of
$551,830.00, from a contract amount of $11,745,545.00 to a total contract amount of
$12,297,375.00. The purpose of the amendment is to develop a crash data entry portal for
towns that do not have an existing means to submit crash data electronically. By providing
a portal for these towns to submit crash data electronically, we will gain comprehensive
crash data insight across NH for input into the predictive Crash Data Analysis System to
be developed by Numetric (the winning vendor for RFP DOS 2026-02 Crash Data Analysis
System). There will be no change in end date.
NOW THEREFORE, in consideration of the foregoing, and the covenants and
conditions contained in the Agreement and set forth herein, the parties agree as follows:

Amendment G will increase the price limitation of the contract for Coforge to develop a
Crash Data Portal so that towns with no RMS system can submit their crash data
electronically.
Statement of Work includes the following deliverables;
• Develop a web-based Portal that is accessible from any modem browser.
• The Portal shall allow for designation of Supervisors to manage users within their
Agency.
• The Portal shall include a work queue Dashboard for the Law Enforcement
Officers and Supervisors.
• The Portal shall be MMUCC 5 compliant, with the ability to upgrade to future
MMUCC versions.
• The Portal shall generate a DSMV Form 159.
• The Portal shall log user-associated transactions for audit purposes.
• The Portal shall be built as a progressive web application, to allowfor crash report
completion while offline.
• Web content must comply with WCAG 2.1, Level A
